AFFIDAVIT

I SRI TOPAN NATH, Age 24 Years, Son of- Sri  Prabhat Nath, resident of Pahumara Nath Gaon, Majuli, Assam, P.O.-Nikinikhowa & Pin- 785104, P.S.- Jengrai mukh, Dist.- Majuli, State- Assam, do hereby solemnly affirm and declared as follows:-

  1. That I am a permanent resident of the aforesaid locality and a citizen of India by birth. This is true to the best of my knowledge, information and belief.
  2. That in my Birth Certificate vide Registration No: 690/15 and Serial No: 0149609 which is issued under Registration of Births and Deaths Act 1969; date of birth has been recorded as 3rd day of March, 1997 (03/03/1997).This is true to the best of my knowledge, information and belief.
  3. That in my HSLC Admit Card date of birth has been recorded as 3rd day of March, 1997(03/03/1997).This is true to the best of my knowledge, information and belief.
  4. That in my Aadhaar Card Vide Aadhaar Card No: 8305 5972 3378 and Pan Card Vide Pan Card No: BXRPN4368R; my date of birth has been erroneously recorded as 1st day of March, 1997 (01/03/1997). This is true to the best of my knowledge, information and belief.
  5. That in my H.S.L.C. Pass Certificate Vide Serial No. 112132780 the date of birth has been wrongly recorded as 1st day of March, 1997(01/03/1997). This is true to the best of my knowledge, information and belief.
  6. That the date of birth as mentioned in the Birth Certificate and H.S.L.C. Admit Card is correct and veracious .This is true to the best of my knowledge, information and belief.
  7. That this Affidavit will be used as a piece of documentary evidence to prove the above mentioned facts and to produce before the concern authority.
